Medical condition

A student who has a health issue of such severity that the student will not be able to attend a local school on a full-time basis for more than 80 consecutive school days may be enrolled with the Longreach School of Distance Education. LSODE will provide a template document to the parent/carer to support application for eligibility under the medical condition category which must be completed by a relevant medical practitioner (registered under the Medical Practitioners Registration Act 2001 and/or registered under the Psychologists Registration Act 2001).

In the case of carer responsibilities please contact the enrolments team for further information.  LSODE will provide a template document to support application for eligibility as a carer which must be completed by a relevant medical practitioner (registered under the Medical Practitioners Registration Act 2001 and/or registered under the Psychologists Registration Act 2001) concerning the requirement of carer.

Your school may provide LSODE with supporting information for fee exemption under the medical category.  Supporting information from your current school may expedite enrolment with fee exemption for medical reasons.  See Medical Enrolments for further information.

Other

The Director-General of Education has the discretion to approve eligibility of other students for whom it is not reasonable to attend a regular school and who do not fit within the preceding sub categories. Applications can be made through the Brisbane School of Distance Education.

A written statement outlining the special circumstance must accompany the enrolment application and be directed to the Principal.

Medical enrolment

This section refers to prospective students with health issues seeking to enrol at LSODE. 

In order to provide a suitable educational program for your child, consultation with the current school of enrolment is encouraged.

For prospective enrolments with fee exemption under the medical category please follow the process below:

Can the student's current school provide information to support fee exemption due to health issues?

Yes 

Current School of Enrolment provides supporting documentation for health issues.

LSODE will consider fee exemption under the medical category and supply Enrolment documents on determination of eligibility - these documents will include a medical certificate proforma supplied by LSODE to be completed by a relevant medical practitioner (registered under the Medical Practitioners Registration Act 2001 and/or registered under the Psychologists Registration Act 2001).

Enrolment Process

  • Unable to attend any schooling option due to medical condition.
  • Possible Internal and/or External agency involvement.
  • Student has identified health issues.
  • Health clinician engaged and able to provide ongoing support to student.
  • Parent/Student willing to engage and has an available Home Tutor.

Referral for fee exemption form completed by school or parent/carer:

  • Can the student's current school provide information to support fee exemption due to health issues? 
  • Medical Category.

Yes

Current School of Enrolment sends supporting documentation to LSODE at Principal@longreachsde.eq.edu.au.

No

Parent or School can contact the LSODE enrolments team and they will direct the enquiry to the relevant officer on (07) 4658 4222 or Principal@longreachsde.eq.edu.au.

Referral (written or verbal) received by LSODE Enrolment:

  • LSODE reviews 'referral for fee exemption - medical category'.
  • On eligibility LSODE enrolments send all required Medical Enrolment Forms to parent/carer for completion.

All LSODE forms returned to LSODE.

Contact with parent regarding subject selection (if required).

Payment of and school costs provided.

Enrolment finalised.

Welcome email received with pertinent information.

Student timetabled and class teacher contact regarding course access.
